ComForCare is a close-knit, growing, home care agency based in Guelph, where we believe exceptional private care starts with trust and real relationships. We are hiring a dedicated Community Care Manager to be the bridge between our clinical services and the wider community.
Are you at your best when you are out in the local healthcare community, building relationships of trust with discharge planners and retirement home staff, while personally guiding anxious families toward the right care plan?
This is a dynamic, hybrid role split evenly down the middle: 50% in-field community outreach, and 50% in-home client assessment, care planning, and family onboarding.
What Your Week Looks Like
- Clinical Assessment & Client Care Management (50%)
- In-Home Clinical Assessments: Meet new clients in their homes to evaluate physical, cognitive, and social needs, drafting comprehensive, person-centred care plans.
- Onboarding & Introductions: Guide families through the intake process and facilitate warm handoffs with our scheduling coordinator and caregiving team.
- Quality Assurance & Reassessments: Conduct follow-up visits as care needs change to ensure family peace of mind and ongoing caregiver alignment.
- Shared On-Call Support: Participate in a balanced, shared on-call rotation with our leadership team for urgent client triage.
- Community Outreach & Partnership Development (50%)
- Build local healthcare bridges: Proactively visit local healthcare and other organizations across Guelph, Cambridge, and Waterloo Region.
- Be our trusted ambassador: Attend community events and meet with partners to keep our agency top-of-mind for care referrals.
- Consultative family intake: Meet with prospective clients and their adult children for initial discovery discussions, explaining private care options with warmth, clarity, and zero hard-sell tactics.
Who You Are
- Registered Practical Nurse (RPN) or Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO).
- Naturally Relationship-Driven: You love conversations, follow up quickly, and build effortless rapport with both older adults and busy healthcare professionals.
- Connected Locally: You already know or have worked within the local healthcare and home care system in Guelph, Cambridge, or Kitchener-Waterloo.
- Self-Directed & Entrepreneurial: You enjoy having real ownership over your daily schedule, balancing outreach mornings with home visit afternoons.
- Road-Ready: You hold a valid G-class driver’s license and have a reliable vehicle for travel across our territory.
Why Join Us?
- Direct Impact in a Local Agency: You won’t be a number in a huge organization. You will work side-by-side with agency leadership, directly influencing our growth and service culture.
- Growth to Full-Time: Option to start at a manageable 20 hours per week, with a structured, transparent timeline to expand into a full-time role (35–40 hrs/week) as client caseload expands.
- Performance Rewards: In addition to a competitive salary, earn commission for every new, ongoing client you bring onto service.
